29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a.  On or about August 20, 2024, at the above addressed jobsite, an employee using a pneumatic nail gun was not wearing safety glasses, thereby exposing the employee to the hazards associated with eye injuries.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a.  On or about August 20, 2024, at the above addressed jobsite, employees performing residential roofing operations, nine (9) feet eight (8) inches, above the ground, were not protected from falls, thereby exposing employees to fall hazards.  b.  On or about August 20, 2024, at the above addressed jobsite, employees performing residential roofing operations, nine (9) feet four (4) inches, above the ground, were not protected from falls, thereby exposing employees to fall hazards.  c.  On or about August 20, 2024, at the above addressed jobsite, employees performing residential roofing operations, sixteen (16) feet six (6) inches, above the ground, were not protected from falls, thereby exposing the employees to fall hazards.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a.  On or about August 20, 2024, at the above addressed jobsite, employees conducting roofing operations were not provided training on how to minimize fall hazards, thereby exposing the employees to the hazards associated with falls.
