29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   At a worksite located at 2329 NE Colonnade Avenue, Blue Springs, MO, employees were exposed to fall hazards.  The employer exposed the employees to these hazards by allowing them to perform roofing work at heights greater than 6 feet without the use of fall protection.  Instances of exposure include but are not limited to:  1.  Three employees exposed to falls greater than 10 feet while performing roofing work near the southern edge of the structures roof.    2.  One employee exposed to falls greater than 12 feet while performing roofing work near the eastern edge of the structures roof.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At a worksite located at 2329 NE Colonnade Avenue, Blue Springs, MO, employees were exposed to fall hazards.  The employer exposed the employees to these hazards by allowing them to utilize a portable extension ladder to access the roof of the structure while the side rails of the ladder did not extend at least 3 feet above the level to which the ladder was being used to gain access.
