29 CFR 1910.147(a)(2)(ii)(B): Lockout/tagout procedures were not implemented when servicing and/or maintenance was taking place during normal production operations when an employee was required to place any part of his/her body into an area on a machine or piece of equipment's point of operation. On or about August 14, 2024, an employee operating a Gloucester 4500 series reclaim extruder was feeding a piece of scrap material into the running machine and did not follow company procedure to use a poly bar but unexpectedly reached their arm into the running machine and contacted the screw auger. was attempting to clear a jam without de-energizing the equipment.
