29 CFR  1910.1200(e)(1):Employer had not developed or implemented a written hazard communication program that included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii):   At a commercial worksite located at 101 Broadway in Excelsior Springs MO -- employees were exposed to, but not limited to, liquid roofing sealant. The sealant has health effects of eye/skin irritation and specific organ toxicity. The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program. This was documented on or about 08/22/2024.

At a commercial worksite located at 101 Broadway in Excelsior Springs MO -- employees were exposed to, but not limited to, roofing sealant. The sealant has health effects of eye/skin irritation and specific organ toxicity. The employer did not develop, implement, and/or maintain at a workplace a written hazard communication program.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  At a commercial worksite located at 101 Broadway in Excelsior Springs MO -- employees were sealing a flat roof and working on the edge of the roof without fall protection. This exposed the employees to fall hazards of approximately 17' to ground level. This was documented on or about 08/22/2024.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At a commercial worksite located at 101 Broadway Excelsior Springs, MO -- employees engaged in sealing of a flat roof were using an extension ladder to gain access. The ladder did not extend 3 feet above the landing surface -- the flat roof being the landing surface. This exposed employees to fall hazards by not having something to hold onto when ascending/descending to/from the roof. This was documented on or about 08/22/2024.
