Location - Jobsite:  On or about 08/22/2024, employees were exposed to falls grater than 6 feet while utilizing life lines connected directly to the front/waist positioning ring and not using D-rings located at the center of back.

Location - Jobsite:  On or about 08/22/2024, employees were exposed to fall hazards while climbing and descending a portable extension ladder that did not extend at least 3 feet above the upper landing.
