(a) 859 Cleveland Ave, Chambersburg, PA; Mixer pit - On or about August 23, 2024, the access ladder into the mixer pit did not extend above the floor level to provide a handhold when entering or exiting the 5-foot-deep mixer pit. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
