LOCATION: The Tower Building worksite at 621 Huntington Ave On or about August 21st, 2024: The Employer exposed workers to numerous hazards when the scaffolding was not altered by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold alteration.
