29 CFR 1926.652(a)(1):Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: On or about August 27, 2024, during the installation of a sewer clean out at a worksite located at 2294 Lake Road, Rocky River, Ohio, the employer failed to ensure an employee working inside a trench, with a varying depth of approximately 6.5 feet (North end) to 7 feet (South end), was protected from cave-ins by an adequately designed protection system such as, benching, sloping, trench shielding, hydraulic shoring, etc. The soil would have been classified as; Previously Disturbed, Cohesive Soil, Type "B".
