29 CFR 1910.36(b)(1): At least two exit routes were not available in the workplace to permit prompt evacuation of employees and other building occupants during an emergency, except as allowed in paragraph (b)(3) of this section. The exit routes must be located as far away as practical from each other so that if one exit route is blocked by fire or smoke, employees can evacuate using the second exit route.  (a) Extraction Laboratory: On or about August 26, 2024, the employer did not provide two means of emergency egress in a high hazard, high occupancy work area to allow for the safe exiting of employees in the event of an emergency.

(a) Extraction Laboratory: On or about August 26, 2024, employees were exposed to the electrical hazard of a flexible extension cord used as a substitute for the fixed wiring of a structure: 1) Above the ceiling in the main room and used to supply power to a power strip powering various equipment on the lab bench.  2) In the extraction room and used to supply power to the Apeks controller unit.

Abatement Note:   Once the PPE Hazard Assessment is completed you shall:   Certify that the hazard assessment has been performed through a written certification in accordance with 1910.132(d)(2), which includes:  1) Identification of the workplace evaluated, 2) The person certifying the evaluation,  3) The date the evaluation was conducted. (a) Extraction Laboratory: On or about August 26, 2024, the employer had not properly documented the locations, including the Extraction Laboratory area where flammable liquids including ethanol were used in the extraction process under the Class and Zone system.
