29 CFR 1910.147(a)(2)(ii)(B):(A) An employee was required to place a part of his or her body into an area on a machine or piece of equipment where work was actually performed upon the material being processed (point of operation) or where an associated danger zone existed during a machine operating cycle. a) Production: On or about December 4, 2020, the employer exposed employees to amputation hazards, in that; the minor servicing activity of cleaning the spray nozzle on the NACHI MC20-01 [AX20], which takes place during normal production and which is routine, repetitive, and integral to the use of the equipment for production, was performed without developing and implementing alternative measures to lockout/tagout that provide sufficient protection.
