29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: (a)Lutheran Social Services of South Dakota dba Canyon Hills Center at 2519 Windmill Drive, Spearfish, SD 57783: On or about and at times prior to August 27, 2024, the employer did not ensure that relocatable power taps were not connected in series (daisy chained) in order to power portable audio/video stations in the eastern wing media room. This condition exposed employees to electrical shock hazards as well as the potential for fire from overloaded circuits.
