29 CFR  1910.147(d)(4)(i):Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:  On or about August 22, 2024 employees engaged in maintenance activities in a meatpacking facility located at 900 South Platte Ave, Fremont, Nebraska 68025 were exposed to amputation and caught between hazards in that the employer failed to ensure employees were isolating hazardous energy by affixing a lock and/or tag.  In this instance a management employee was observed attempting to dislodge equipment that was jammed between two adjoining conveyor belts on the MODA #6 production line without affixing his LOTO device prior to attempting to free the jam.
