(a) On or about August 29, 2024, at 1301 Edgewood Ave, South Milwaukee, WI 53172, the employer did not ensure employees conducting installation of a new residential roof, using hand tools and nail guns were wearing eye protection. This exposed the employees to struck by hazards and eye injuries.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). (a) On or about August 29, 2024, at 1301 Edgewood Ave, South Milwaukee, WI 53172, employees installing a roof on an existing residential home roof with an approximately 11:12 pitch were not using fall protection. This exposed employees to a fall hazard of approximately 22 feet 11 inches from the roof eve to the ground below on the back (south) side of the home.
