29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a) On or about 8/29/2024, at 5998 Calm Lake Dr., Farmington NY 14425 : Employees were installing roofing material on a house with an estimated 5:12 pitch roof without fall protection. Employees were exposed to a fall of approximately 16 feet.  Navarro Roofing Inc. was previously cited for an equivalent violation of the safety and health standard 1926.501(b)(11), which was contained in OSHA inspection 1457058, citation number 1, item number 1 and was affirmed as final order on 2/24/2020, with respect to a workplace located at 3650 Lake Road North, Brockport NY.

(a) On or about 8/29/2024, at 5998 Calm Lake Dr, Farmington NY, 14425: Previously trained employees were working on a roof in excess of 16 feet above the ground without fall protection of any kind and exposed to a fall hazard.

29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access nor was the ladder secured at it's top to a ridged support and a grasping device, such as a grabrail, provided to assist the employee in mounting and dismounting the ladder:  (a) On or about 8/29/2024, at 5998 Calm Lake Dr, Farmington NY, 14425: Employees were using an extension ladder to access the roof and the ladder was not extended 3 feet above the roofs edge.
