29 CFR  1910.147(c)(4)(i):Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.  (a): On or about September 13, 2024, the employer did not develop, document, and utilize energy control procedures prior to allowing employees to perform servicing and maintenance activities on Pouch Line #13.  Employees were exposed to caught-in and amputation injuries while performing adjustments to the equipment.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.  (a): On or about August 29, 2024, the employer did not ensure that machinery points of operation were guarded to protect employees from contact with the danger zone during the operating cycle.  Employees operating Pouch Line #3 were exposed to amputation hazards from the sheer blade while operating.  (b): On or about August 29, 2024, the employer did not ensure that machinery points of operation were guarded to protect employees from contact with the danger zone during the operating cycle.  Employees operating Pouch Line #6 were exposed to amputation hazards from the sheer blade while operating.  (c): On or about August 29, 2024, the employer did not ensure that machinery points of operation were guarded to protect employees from contact with the danger zone during the operating cycle.  Employees operating Pouch Line #12 were exposed to amputation hazards from the sheer blade while operating.  Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.  (a): On or about August 29, 2024, the employer did not ensure that machinery points of operation were guarded to protect employees from contact with the danger zone during the operating cycle.  Employees operating Press Machine #2 were exposed to amputation hazards from the incoming nip points of rollers while operating.  (b): On or about August 29, 2024, the employer did not ensure that machinery points of operation were guarded to protect employees from contact with the danger zone during the operating cycle.  Employees operating Press Machine #3 were exposed to amputation hazards from the incoming nip points of rollers while operating.  (c): On or about August 29, 2024, the employer did not ensure that machinery points of operation were guarded to protect employees from contact with the danger zone during the operating cycle.  Employees operating Slitter #1 were exposed to amputation hazards from the incoming nip points of rollers while operating.  Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR  1910.305(g)(2)(ii):Flexible cords may be used only in continuous lengths without splice or tap. Hard-service cord and junior hard-service cord No. 14 and larger may be repaired if spliced so that the splice retains the insulation, outer sheath properties, and usage characteristics of the cord being spliced.  (c): On or about August 29, 2024, the employer did not ensure that flexible cords were used in continuous lengths without splice near Pouch Machine #6, exposing employees to shock hazards.  A LED work lamp was modified to accept a two-prong flexible cord.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
