At a residential worksite located at 4313 Palisade Ct, Riverside MO -- employees were exposed to caulk/adhesive and gasoline. The employer did not compile a list of hazardous chemicals used at the jobsite. This was documented on or about 08/27/2024.

29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  At a new residential worksite located at 4313 Palisades Ct, Riverside MO -- employees were working below roofers without head protection. The employees were picking up trash/debris from around the worksite while employees were engaged in roofing duties working above. This exposed the employees on the ground to head injuries from tools, materials, etc., falling on their heads. This was documented on or about 08/27/2024.

29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee was using eye or face protection when exposed to eye or face hazards from flying particles.  At a new residential worksite located at 4313 Palisade Ct, Riverside MO -- employees engaged in roofing duties using pneumatic nail guns, hammers and other various small hand tools were not wearing eye protection. This exposed the employees to shrapnel and debris to get in their eyes causing injury. This was documented on or about 08/27/2024.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   At a new residential worksite located at 4313 Palisades Ct, Riverside MO -- employees engaged in roofing duties were not using any personal fall arrest system. This exposed the employees to fall hazards causing serious injury and/or death. This was documented on or about 08/27/2024.  Alexandros Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(11), which was contained in OSHA inspection number 1487888, citation number 1, item number 1 and was affirmed as a final order on 03/01/2021, with respect to a workplace located at 4101 S MO Hwy 7, Blue Springs, MO.
