Employees were exposed to 16-24.1 foot falls from a low-sloped roof deck. Note* Abatement certification AND supporting documentation of correction are required for this item.

29 CFR 1926.405(b)(1): Conductors entering boxes, cabinets, and fittings were not protected from abrasion, and openings through which conductors enter were not effectively closed: (a) Rock Bottom Dairy Pole Barn: Workers were using a rented SkyJack scissor lift (SJ6826RT 4x4) during metal decking installation activities. The fitting for the flexible cable entering the operator control box was broken/separated. This exposed the individual wires to damage from the opening. NOTE: Certification of corrective action is required for this item.

The fitting for the flexible cable entering the operator control box was broken/separated. The individual wires applied pull directly to the terminals. NOTE: Certification of corrective action is required for this item.
