29 CFR 1910.95(g)(6):At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels : a) Production Area Hot End: An IS Machine Operator was exposed to continuous noise levels in excess of the allowable 8-hour time-weighted average sound level (Permissible Exposure Limit). The employee's dosimeter dBA level was 678.1% of the permissible level (90 dBA) and was equivalent to approximately 103.8 dBA. The sampling was performed for 352 minutes during one shift on 9/11/24. Zero exposure was assumed for the unsampled period of time. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

East Ave Bridgeton, NJ: An audiometric exam issued on 3/27/24 a maintenance supervisor indicated a STS that was confirmed by a rested on 4/11/24 and was not recorded on the OSHA 2024 300 log within seven calendar days. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
