29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) 3026 E 142nd Dr., Thornton, CO: On or about September 3, 2024, and at times prior, employees were exposed to falls of approximately 20 feet to the ground below while conducting roofing activities on an existing residential home. The employer failed to provide training on items such as, but not limited to; nature of fall hazards in the work area; correct procedures for inspecting the fall protection systems to be used; use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used; limitations on the use of mechanical equipment during the performance of roofing work.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) 3026 E 142nd Ave., Thornton, CO: On or about September 3, 2024, and at times prior, as the exposing employer, Loyal Construction Inc., did not ensure employees were protected in that employees were exposed to a fall hazard of approximately 20 feet to the ground below.  Loyal Construction Inc., was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1501018 , citation number 001, item number 001 and was affirmed as a final order on December 15, 2020, with respect to a workplace located at 7349 Steward Lane, Colorado Springs, CO 80922.
