OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:  a) For the Dorrco Phosacid reactor and flash cooler at Mosaic Riverview:  On or about 08/29/2024, the employer exposed employees to caught-in hazards, in that, a hydro-blasting hose was not protected from being entangled in a rotating agitator shaft. This resulted in the potential for employees to be wrapped by the hose and/or pulled into/against the agitator and housing.
