29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. On or about August 28, 2024, at 3725 Foundry Way in St. Louis, MO, employees were exposed to fall hazards while working around a hole designed for a trash chute. An employee was insulating the walls around the area when he fell through the hole and landed three stories, approximately 30 feet, below.
