[D] where tag-out is used for energy control, include a review between the inspector and each authorized and affected employee, of that employee's responsibilities under the energy control procedure being inspected, and the elements set forth in paragraph (c)(7)(ii) of this section.  a)  9775 Commerce Circle Kutztown Pa, Press Room Floor - On or about September 3, 2024, an employee was performing maintenance on a printing press.  The machine was not de-energized, exposing the employee to the machine's rotating parts.  The employer last inspected the energy control program in 2019.  b)  9775 Commerce Circle Kutztown Pa, Press room Floor - On or about September 4, 2024, two employees were performing maintenance on a printing press line without de-energizing the equipment, exposing the employees to unguarded rotating parts from unintentional start up.  The employer last inspected the energy control program in 2019.
