29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  On or about August 29, 2024, the employer did not instruct employees in the recognition or avoidance of hazards and avoidance of unsafe conditions to include the danger of electrocution hazards while working around or near low hanging primary electrical lines. On August 29, 2024, an employee, while vertically carrying a fully extended 14-foot aluminum extension ladder, made contact with an energized low hanging primary electrical line resulting in electrocution/in-patient hospitalization while completing roofing work at residential home located at 6429 Lake Road Saybrook TWP, Ohio 44004.

29 CFR 1926.416(a)(1): The employer permitted employees to work in close proximity to energized, unguarded, and ungrounded electric power circuits that the employee could contact in the course of their work:   On or about August 29, 2024, the employer did not ensure low hanging power lines were de-energized and permitted employees to work within a close proximity during the re-shingling roof work completed at a residential home located at 6429 Lake Road Saybrook TWP, Ohio 44004.  As a result, an employee, while vertically carrying a fully extended 14-foot aluminum extension ladder, made contact with an energized low hanging primary electrical line resulting in electrocution/in-patient hospitalization
