(a) On or about September 4, 2024, at 1409 Edgewood Ave, South Milwaukee, WI 53172, employees installing a roof on an existing residential home roof with an approximately 8:12 pitch were not using fall protection. This exposed employees to a fall hazard of approximately 22 feet 3 inches from the roof eve to the ground below on the left (east) side of the home.

(a) On or about September 4, 2024, at 1409 Edgewood Ave, South Milwaukee, WI 53172, employees were accessing the roof to conduct roofing work from a Werner extension ladder which did not extend at least three (3) feet above the upper landing surface, exposing employees to fall hazards of approximately 17' 4" on the back of the home.
