29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  On or about September 4, 2024 and at times prior thereto, on the south end of the building, an employee was not in use of fall protection while performing roofing work activities in the area between the warning line and the edge of the roof that was more than six(6) feet to the ground below.

29 CFR  1926.502(f)(1)(i): When no mechanical equipment was being used the warning line was not erected at least 6 feet (1.8m) from the roof edge.   On or about September 4, 2024 and at times prior thereto, on the south end of the building roof, employees were performing roofing work activities on a roof that was more than six(6) feet in height with the warning line laying on the roof less than six(6) feet from the edge.

On or about September 4, 2024 and at times prior thereto, on the south end of the building, an employee was performing roofing work activities from the roof of a building more than six(6) feet in height and was not trained to recognize the hazards of falls and the use of fall protection.

On or about September 4, 2024, on the south end of the central mall shopping center, the PIT operator left the PIT unattended with the forks in the elevated position.
