29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. On or about September 4, 2024 and at times prior thereto, on the south end of the building, an employee of a subcontractor was not in use of fall protection while performing roofing work activities in the area between the warning line and the edge of the roof that was more than six(6) feet to the ground below.

29 CFR 1926.502(f)(1)(i): When no mechanical equipment was being used the warning line was not erected at least 6 feet (1.8m) from the roof edge. On or about September 4, 2024 and at times prior thereto, on the south end of the building roof, employees of a subcontractor were performing roofing work activities on a roof that was more than six(6) feet in height with the warning line laying on the roof less than six(6) feet from the edge.

29 CFR 1926.503(c)(3): Employees were not retrained when inadequacies in knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill. On or about September 4, 2024 and at times prior thereto, on the south end of the roof, employees who are to identify the inadequacies of fall protection being used were not retrained to recognize the hazards of falls and employees of a subcontractor performing roofing work activities were not in use of fall protection while working at heights greater than six(6) feet.
