OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the employees were exposed to struck-by and crushing hazards from collapsing storage racks due to missing bolts to secure the racks: (a) Establishment, Warehouse area - On or about September 3, 2024, for employees require to operate or work alongside of the operation of a forklift placing and retrieving pallets on the steel storage racks. The employer did not ensure that all rack columns were anchored to the warehouse floor with anchor bolts.
