29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Maintenance Shop - On or about August 23, 2024, an employee was exposed to crushing hazards when operating a Shearing Machine (Model: US825, Serial # J1402 - 012) to cut a small part (Tuffak Polycarbonate) approximately 5.5 inches in length x 4 inches in width x 0.25 inches thick without adequate guarding in place.
