Location: 78 Horseblock Road, Yaphank, NY 11980. On or about 09/05/2024, The employer did not tag or remove a damaged 16 feet Uline Rolling ladder that employees used to access merchandise from shelving racks. NOTE: Because abatement of this violation is already documented in the inspection casefile, the employer need not submit certification or documentation of abatement for this violation as normally required by 29 CFR 1903.19.

On or about 09/05/2024, Employees who transfer auto parts on the mezzanine level at 11.5 feet above the ground were not protected from falling. The mezzanine level had an open sided edge and was missing a guardrail system. Note: Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.
