(a) Facility at 35 Stilson Road in Wyoming, Rhode Island: On or about 9/5/2024, employees were exposed to injurious corrosive materials, including but not limited to Dissolve-a-Krete with a pH of <1.8, while dispensing into containers used to clean the trucks and equipment without access to immediate, suitable and adequate drenching or eye flushing facilities.
