29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section. SITE LOCATION/ALLNEX USA, INC., 528 SOUTH CHERRY STREET, WALLINGFORD, CT 06492: On/or about August 30, 2024, the employer did not establish site specific hazardous energy control lockout/tagout procedures for the employees on the high powered water pressure cleaning such as but not limited to the Kettle (K61), foot pedal, operated lance(s), pressurized truck(s) which created the unexpected energization of the anti-withdrawal lance where the employee was struck by the pressure of the water jet resulting multiple water jet punctures.
