29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Storm window line, saw 1; On or about August 26, 2024, and dates prior, adequate point of operation guarding was not provided on a Sampson double miter saw (serial no. 11317) being used to cut aluminum extrusion. (b) Storm window line, saw 2; On or about August 26, 2024, and dates prior, adequate point of operation guarding was not provided on a Sampson model SC-12 straight cut saw (serial no. 91032) being used to cut aluminum extrusion. (c) Storm window line, saw 2; On or about August 26, 2024, and dates prior, employees were not protected from a pinch point created by the pneumatic action when the Sampson model SC-12 straight cut saw (serial no. 91032) returns from the lowered cutting position to the upward position.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: (a) Storm window line, saw 1; On or about August 26, 2024, an employee's right middle finger and right index fingertip were amputated while operating a Sampson double miter saw (Serial No. 11317) to cut aluminum extrusion, which was not reported to OSHA until August 28, 2024.
