(a) Anchor Glass Container Corporation, 151 East McCanns Boulevard, Elmira Heights, NY 14903, on or about 8/30/2024- Employees working from scaffolding were assigned to patch a hole on a batch bin in the Coal Building. The scaffolding did not meet the requirements of 29 CFR �� 1926.45l(f)(3) and (f)(15), in that a competent person did not complete an inspection for visible defects before the work shift and a stepladder was used on the scaffold to increase the working height of employees without providing for additional fall protection. Abatement certification is required.
