29 CFR 1910.147(a)(2)(ii)(B): Employer failed to provide Alternative Protective Measures for employees who are required to place any part of his or her body into an area on a machine or piece of equipment where work is actually performed upon the material being processed (point of operation) or where an associated danger zone exists during a machine operating cycle. a) Press 117 On or about , August 27, 2024, and at times prior, the employer exposed employees to Amputation Hazards, in that alternate protective measures for LOTO were not provided to employees performing maintenance on Press 117. Employees performs live (machine under power) change out of various dies, which includes installing and removing dies.
