29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service: (a) On or about September 6, 2024, employees were exposed to electrical shock hazards when using an electrical cord with an outer sheath that had been pulled from the fittings on the control box, which provided a 120 V connection to the automatic ejection system that controlled the part ejection from a centerless grinder (Model No. 20M, S/N 2M2H1Z-355). No abatement certification or documentation is required for this item.
