29 CFR  1910.147(c)(6)(i):The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.  A) 551 Raritan Center Parkway, Edison NJ/Icy Cool Machine   The employer exposed employees to potential hazardous electrical energy in that the employer did not conduct annual audits of the energy control procedure.  On or about September 9, 2024.

A) 551 Raritan Center Parkway, Edison NJ  08837  The employer did not provide adequate training on use of energy control procedures to machine operators.  An affected employee did not receive lockout/tagout training before working on the Icy Cool machine.     On or about September 9, 2024.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  A) 551 Raritan Center Parkway, Edison NJ  08837  The Icy Cool machine was not guarded exposing employees to caught-in hazards.   The short seals of the Icy Cool machine were not guarded to prevent employee contact.   On or about September 9, 2024.
