29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary. (a) Worksite: On or about 09-10-2024, employees were exposed to fall hazards while working from a 24-foot Alum-A-Pole pump-jack scaffold that was not secured to the structure at the bottom causing the scaffold to slip outward from the bottom, during roof shingle activities at a height of approximately 15 feet 3 inches above the lower level without any means of fall protection.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): (a) Worksite: On or about 09-10-2024, an employee was exposed to fall hazards while removing and installing roof shingles, at a height of approximately 25 feet above the lower level, without any means of fall protection.
