29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). (a) On or about September 10, 2024, at 1400 E. Elm Rd, Oak Creek, WI 53154, employees installing a roof on an existing residential home roof with an approximately 4:12 pitch, were not using fall protection. This exposed employees to a fall hazard of approximately 8 feet 8 inches from the roof eve to the ground below on the right (east) side of the home.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. (a) On or about September 10, 2024, at 1400 E. Elm Rd, Oak Creek, WI 53154, employees were accessing the roof to conduct roofing work from two different ladders (front of home and back of home) which did not extend at least three (3) feet above the upper landing surface, exposing employees to fall hazards of approximately 8ft 8in on the back of the home.
