Location: 434 York Place, West Babylon, NY 11704 On or about 09/11/2024, Employees were using a Werner 32 ft extension ladder to access a ladder jack platform scaffolding plank. The ladder only extended 1.5ft above the landing. Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
