29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii) a) Jobsite located at or near 3618 Wallace Street, Philadelphia, PA- On or about September 11, 2024, the employer did not ensure that each working level of a scaffold was fully planked or decked between the front uprights and guardrail supports. Employees conducting masonry activities were utilizing a scaffold platform that was not fully planked, thereby exposing employees to fall hazards or approximately 25 feet.

Employees worked on the scaffold without fall protection systems or guardrail systems, thereby exposing employees to fall hazards of approximately 12 to 25 feet.
