Employees conducting masonry activities were utilizing a scaffold platform that was not fully planked, thereby exposing employees to fall hazards or approximately 25 feet.

Employees worked on the scaffold without fall protection systems or guardrail systems, thereby exposing employees to fall hazards of approximately 12 to 25 feet.
