29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a) On or about September 11, 2024, at a jobsite located at 1435 Edgewood Ave. South Milwaukee, WI 53172, one of the portable ladders being used to access the roof of the structure did not extend at least three feet above the upper landing area nor was it secured in place. This exposed employees accessing and egressing to and from the roof, to a fall hazard of approximately  8.8 feet onto soft ground.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) On or about September 11, 2024, at a jobsite located at 1435 Edgewood Ave. South Milwaukee, WI 53172, employees conducting roofing activities (shingle installation) on a 4:12 pitch roof of the structure were not protected from falling to the lower level. This exposed employees to a fall hazard from the first floor roof of approximately 8.8 feet to 16 feet on the North side of the structure.  J&J Contractors I, LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1656776, citation number 1, item number 1 and was affirmed as a final order on May 9, 2023, with respect to a workplace located at W278 N6145 Serenity Dr., Merton, WI 53056.
