Location: 6 Captain James Mann Memorial Drive Cromwell, Connecticut On or about September 11, 2024, an employee was exposed to struck-by and laceration hazards while operating a Husqvarna K770 portable gasoline powered saw while cutting unsupported concrete resulting in severe facial trauma.

Location: 6 Captain James Mann Memorial Drive Cromwell, Connecticut On or about September 11, 2024, the employer did not fully and properly implement the engineering controls and work practices specified for the task on Table 1 while employees were cutting a concrete precast basin outdoors using a Husqvarna K770 demolition saw equipped with a 14 inch diamond cutting blade.
