29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered. a) On or about September 11, 2024, the employer exposed employees to struck-by and crushing hazards, in that the Linde forklift (model H25T, Serial number H21202Y05053) was not marked to identify the Bolzoni Auramo clamp attachment and show the approximate weight of the truck and attachment combination at maximum height with the load laterally centered. b) On or about September 11, 2024, the employer exposed employees to struck-by and crushing hazards, in that the Toyota forklift (model 8FBCHU25, Serial number 68131) was not marked to identify the RightLine clamp attachment and show the approximate weight of the truck and attachment combination at maximum height with the load laterally centered.
