29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or about September 11, 2024, employees conducting roofing work on an existing residential property with an 11:12 pitch roof, were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 12 feet on the front side of the property and 19 feet on the back side of the property to the ground below.

29 CFR 1926.503(c)(3): 29 CFR 1926.503(c)(3): When inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill, the employer shall retrain the affected employee. (a) On or about September 11, 2024, the employer did not ensure employees exposed to potential fall hazards had been properly trained to recognize and minimize the fall hazards associated with their work, and how to properly install anchor points and ensure proper selection of fall protection equipment.
