29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. a. On or about July 25, 2024, the employer failed to provide effective guarding to protect employees working at or near the guide belt from rotating parts on the 18" cutting line prior to the glass conveying into the second grinder. A machine operator running production was exposed to rotating parts while their left-hand index finger was caught in the guide belt and pulley, resulting in a crushing injury to their left index finger.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed: a. On or about September 11, 2024, the employer failed to provide effective guarding to protect employees working at or near the transfer conveyor from a rotating chain and sprocket on the 18" cutting line prior to the glass entering the first grinder. A machine operator standing at the exit end between the #2 breaker and glass transfer table was exposed to rotating parts during normal production operations when their hands were approximately six inches (6") away from an unguarded chain and sprocket.
