(a)Cone AG, Inc. at 2000 South 4th Street, Onida , SD 57564: On or about and at times prior to September 11, 2024, the employer did not ensure that brick systems utilized as stairs to enter and egress from the berm of the Liquid Fertilizer area had uniform riser heights and tread depths between landings. This condition exposed employees to slip, trip and fall hazards associated with non-uniform walking/working surfaces. (b)Cone AG, Inc. at 2000 South 4th Street, Onida , SD 57564: On or about and at times prior to September 11, 2024, the employer did not ensure that the homemade wood stair system utilized to enter and egress from the control room to the berm of the Liquid Fertilizer area had uniform riser heights and tread depths between landings. This condition exposed employees to slip, trip and fall hazards associated with non-uniform walking/working surfaces.
