29 CFR 1910.145(f)(3): Tags were not used as a means to prevent accidental injury or illness to employees who were exposed to hazardous or potentially hazardous conditions, equipment or operations which were out of the ordinary, unexpected or not readily a.) Modern Industries, Inc., laboratory - On or about 9/10/24, and at times prior there to, the company did not tag a hoist when an unsafe condition was found. The hoists included, but were not limited to, 1 ton Gorbel hoist, that had a loose screw in the base plate. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

Modern Industries, Inc. - The company did not provide the OSHA 300 log for Calendar Year 2024 within four hours of when it was requested. The log was requested on 9/12/24 and as of 12/17/24 the log was still not received. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).
