29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle. a. On or about September 4, 2024, in the Builders area, employees were able to reach under the safety guard to access the point of operation on the Castle Screw Pocket Machine Model TSM-21, Serial #51484 during wood working operations. Employees were thereby exposed to crushing and amputation hazards associated with an unguarded point of operation.
