29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  a) Worksite, 971 North Colebrook Road Manheim, PA - On or about September 12, 2024, employees worked inside an elevated Genie Model# S-60X Articulating Boom Lift basket at a height of approximately 20 feet above grade without fall protection.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  a) Worksite, 971 North Colebrook Road Manheim, PA - On or about September 12, 2024, each employee who worked inside an elevated Genie Model# S-60X Articulating Boom Lift basket did not receive training in appropriate fall protection methods to avoid fall hazards associated with the work. 29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) Worksite, 971 North Colebrook Road Manheim, PA - On or about September 12, 2024, employees performed roofing activities on a commercial structure with a 5:12 roof pitch without fall protection and were exposed to fall hazards up to approximately 34 feet to the grade below.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
